If you’ve ever wondered whether your hormones are out of balance, here’s a good way to do your own “detective work” to find out! Click on the button below to download our free Symptom Tracker. It’s a checklist of symptoms and health issues that are potential signs you may have a hormone imbalance. Each day for a month, use the Symptom Tracker to keep a log of the symptoms you had that day. Rate your symptoms on a scale of 0 to 5 for the day so you can easily see over time how you’re doing.

  • 0: Not a problem

  • 1: Rarely a problem

  • 2: Bothered me occasionally

  • 3: Regularly a problem

  • 4: Almost always a problem

  • 5: Constantly a problem

If you start using bioidentical hormones to balance your hormones, you can also use the Symptom Tracker to determine whether your symptoms become less over time.

If you have problems downloading the PDF file, you can also “right click” on the image of the Symptom Tracker that appears below the download button, then click on the “Save As…” option to save the image to your computing device. You can then open and print the image.
